We believe in supporting the well-being, independence, and community connection of older adults across Chicago. Our programs are designed to offer essential services and meaningful experiences that enhance quality of life, encourage active engagement, and provide the care our participants need to live comfortably and safely.

What We Offer?

Personalized Support & Advocacy

Our Intensive Case Advocacy & Support (ICAS) and Community-Based Case Management programs are tailored to provide essential assistance and advocacy to older adults who need help with resources, meal access, and financial management. Through one-on-one case management, we help each individual feel secure, supported, and empowered to age comfortably in their own home.

Friendly Connections

Social connection is vital for well-being. Through our Companion Services, we connect older adults who feel isolated with trained volunteers for friendly visits. These companions provide companionship, friendship, and a comforting presence to support mental and emotional health.

Monthly Gatherings

For mobile and active older adults on the West Side, our “Take Charge” program offers a monthly opportunity to gather, socialize, and learn. Each meeting includes a shared meal, engaging speakers, and educational topics, along with occasional day trips to enriching destinations like museums or local cultural sites.

Home Delivery

To support older adults with limited mobility, our Home Delivery Program brings essential groceries and regular well-being checks right to their door. Each month, older adults in our service area receive fresh produce, protein, and helpful resources, allowing them to stay comfortably in their homes with peace of mind. We focus on serving the West Side community within our established boundaries.

Service Area for Home Delivery:

  • East to Racine, West to the City limits
  • North to Bloomingdale, South to Cermak

While our Home Delivery Program often reaches capacity, we maintain a waitlist for those interested.
